Conflict Free Case Management

ISN’s Case Management Team provides coordination and oversight of services for individuals over the age of 21 with Intellectual & Developmental Disabilities or an Acquired Brain Disorder. ISN works collaboratively and in close partnership with local Area Agencies to offer Conflict Free Case Management to eligible and interested individuals.

At the core level Conflict-Free Case Management (CFCM) requires that assessment and coordination of services are separate from the delivery of services, with the goal to limit any conscious or unconscious bias a care manager or agency may have, and ultimately promote the individual’s choice and independence.

At this time ISN is offering Conflict Free Case Management services to the greater areas of the following counties: Sullivan, Hillsborough, Merrimack and Rockingham. However, we welcome referrals from anywhere in New Hampshire.

Types of Case Management Services Offered:

Traditional
Individuals receiving: Residential, Day, Supported Employment and/or Community Support Services
Family Support
Individuals living at home with a family member
Participant Directed & Managed Services (PDMS)
Individuals and families living together who manage their own services and budget
